I worked for over 10 years as a movie theatre manager (shudder). I can tell you that we pretty much ignored those ratings as much as parents did.
The only time I worried about an R rating was when I feared the idiot kids would ruin a movie for adults (Schindler's List is a prime example). Otherwise, most R rated movies were a geared for kids (read: any horror flick).
I was always surprised at parents that would buy their kids tickets to some of the most violent R rated films. More amazing is that their only question would be "Is there sex in this film?" Decapitation was fine, breasts were not.
